Requests should be made using the white repeat prescription slip on the right side of your prescription, ticking only those items that are required. You may need to see or speak to a doctor for items not on that list. You can also fill in a form available at reception to make a written request if you do not have the usual repeat slip.

Reception staff cannot take requests for repeat prescriptions over the phone. This is due to risk management and is necessary to avoid mistakes in your medicine. In situations when it is very important to obtain medication without making a written request in the normal way, you will need to speak to a doctor.
Please give 48 hours notice for repeat prescriptions.
You can collect your prescription or we will post it to you if you provide a stamped addressed envelope.
